I'm building pretty much the same thing - Here's the link to my project log.
http://rampplans.org/forums/?board=ramp ... 1236132829

Please realize how fast this ramp will be.
You can make the transition mellower but it will make the total length more.

Transition: 40 inches.
Ramp height: 24 Inches.
Platform length: 9 Inches.
Flat bottom length: 28 inches.
Flat bottom height: 1 inch.
Which would make the total length 10.01 feet and the total height 2.08 feet

Materials you will need
Quantity Material
50 4' 2x4
3 4' x 4' sheet of 3/4" plywood
6 4' x 4' sheet of 3/8" plywood
6 4' x 4' sheet of 1/4" Masonite
2 4 feet long 1.5" diameter metal pipe

you can probably get Masonite at any hardware store that sells plywood/2x4s or any lumber yard

This is what your ramp will look like when completed
